DISCLAIMER - MUST READ: I declare that my child is medically fit to participate in training with HESA and I understand in doing so, it is at their own risk. Any injury sustained to any athlete during, or as a consequence of the sports/ activities, HESA will not be held liable, for any loss, claims, costs or expenses which may arise from the postponement or cancellation of training for any reason. I also acknowledge the fact that some of the activities/ exercises may require physical assistance from the coaches. I understand that HESA strongly advise that if I have not participated in regular intense physical activity prior to beginning training that I should seek advice from a doctor for my child. If any new medical conditions arise, I must inform my coach and Amanda Croall before continuing with training. I hereby state that I have read, understood and answered honestly the questions above. I also state that I consent to my child participating in activities, which will include aerobic exercises such as running, jumping, lifting, resistance exercises (including but not limited to barbells, dumbbells, kettlebells and various weight training equipment) and stretching along with physical contact team sports. I realise that my child's participation in these activities involves the risk of injury and even the possibility of death. Furthermore, I hereby confirm that I am voluntarily engaging my child in an acceptable level of exercise & training, which has been recommended to them, and that I should consult a doctor if they are suffering from any condition that might make physical activity injurious to their health. I understand the results of any fitness program cannot be guaranteed and my child's progress depends on their effort and cooperation inside and outside of the sessions. I understand that photographs and videos may be used for promotional purposes and I agree for them to be used. I agree that HESA shall not be liable or responsible for any injuries to my child resulting from their participation in the sports performance fitness sessions (whether at home, online, in the studio, outdoors or in any corporate / commercial facility) and I expressly release and discharge HESA and it's owners/ employees/ coaches from all claims, actions, judgements and the like which I or my heirs may have or claim to have as a result of any injury or other damage which may occur in connection with my child's participation in the HESA academy program.
